RISE

Recognition, Intervention, Shared Decision-Making, and Education

Launching January 2027

What Is RISE?

RISE is Empira's newest collaborative quality improvement initiative, launching in January 2027 with nine long-term care communities from Cassia and Saint Therese.

The program's primary goal is to reduce avoidable hospitalizations by strengthening clinical practices, improving communication, and supporting timely, person-centered care within long-term care communities.

The RISE Strategies

RISE will focus on four interconnected strategies:

  • Recognition: Identifying and assessing changes in resident condition early.

  • Intervention: Supporting timely, evidence-informed clinical responses through tools such as INTERACT.

  • Shared Decision-Making: Ensuring care decisions reflect residents' goals, preferences, and wishes through meaningful collaboration with families and healthcare providers.

  • Education: Strengthening staff knowledge, clinical confidence, and interdisciplinary teamwork.

Through these strategies, RISE aims to improve care coordination, reduce unnecessary hospital transfers, and strengthen the ability of long-term care teams to manage changes in resident condition.

RISE will focus on delivering the right care, at the right time, in the right setting, while honoring what matters most to each resident.
